Responsibilities:

  1. Mentorship: Provide individualized mentoring and support to young adults by fostering trusting relationships, promoting personal growth, and encouraging self-reliance.

  2. Life Skills Development: Facilitate the acquisition of essential life skills such as budgeting, time management, communication, job readiness, and problem-solving.

  3. Case Management: Collaborate with a multidisciplinary team to assess participants' needs, develop personalized treatment plans, and ensure consistent progress through regular check-ins and evaluations.

  4. Goal Setting: Assist participants in setting and achieving personal, educational, and career goals by providing guidance, resources, and accountability.

  5. Emotional Support: Offer compassionate and empathetic support to participants, assisting them in managing stress, building resilience, and fostering healthy coping strategies.

  6. Crisis Intervention: Be available to handle crisis situations, offering appropriate support, and implementing intervention strategies as needed.

  7. Advocacy: Advocate for participants' needs within the community and coordinate with external agencies, educational institutions, and healthcare providers to ensure comprehensive care.

  8. Documentation and Reporting: Maintain accurate and up-to-date participant records, including progress notes, treatment plans, and outcome measures, while adhering to confidentiality and privacy regulations.

  9. Team Collaboration: Collaborate with colleagues, including therapists, clinicians, and program managers, to ensure seamless coordination of care and effective treatment outcomes.

  10. Professional Development: Engage in ongoing professional development opportunities to enhance knowledge of evidence-based practices, mentorship techniques, and relevant industry trends.

Qualifications:

  • Bachelor's degree in psychology, social work, counseling, or a related field

  • Prior experience in mentoring, coaching, or case management, preferably with young adults or individuals in transition.

  • Strong interpersonal skills with the ability to build trusting relationships and communicate effectively with participants, families, and professionals.

  • Knowledge of mental health, substance use disorders, and trauma-informed care principles.

  • Understanding of life skills development and the challenges faced by young adults in transition.

  • Excellent organizational skills, with the ability to prioritize tasks, manage time effectively, and meet deadlines.

  • Strong problem-solving abilities and the capacity to adapt to evolving situations.
